Bloom by Roze Health provides telehealth services exclusively to patients who are physically located in the states of Colorado and Wyoming at the time of each consultation. By enrolling, you confirm your physical presence in one of these states and agree to notify us if you relocate.
Bloom by Roze Health offers telehealth-based metabolic wellness services, including GLP-1 therapy management and related clinical support. Our services are provided by a licensed Nurse Practitioner (FNP-C) operating within the scope of applicable state law.
Our services are not a substitute for in-person care where in-person care is clinically indicated. Bloom does not provide emergency medical services.
If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Do not use our telehealth platform to seek emergency care.
Membership fees are billed on a monthly or agreed-upon recurring basis beginning at enrollment. By enrolling, you authorize Bloom to charge your payment method on file for all applicable fees.
Fees are subject to change with reasonable notice. Continued enrollment after notice of a fee change constitutes acceptance of the updated pricing.
Where clinically appropriate, Bloom may prescribe medications through our integrated ePrescribing platform (Practice Better). Prescriptions are filled through our partnered compounding pharmacy. Bloom does not guarantee specific medications will be prescribed or that insurance coverage will apply.

To the maximum extent permitted by applicable law, Bloom by Roze Health, LLC, its owners, employees, and contractors shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of or inability to use our services, including but not limited to personal injury, loss of data, or financial loss.
Our total liability for any claim arising under these Terms shall not exceed the amount you paid to Bloom in the three months preceding the claim.
All content on our website — including text, images, logos, and downloadable materials — is the property of Bloom by Roze Health, LLC and is protected by applicable copyright law. Reproduction or redistribution without written permission is prohibited.
These Terms shall be governed by the laws of the State of Colorado, without regard to its conflict of law provisions.
We reserve the right to update these Terms at any time. The effective date will reflect the most recent revision. Continued use of our services constitutes acceptance of updated Terms.
